AOS Office Management Forms by Kay Gerety

The following are several examples of forms, letters, agreements, contracts, reports etc. to simplify the incorporation of orthodontics into your practice. One of the most trying aspects about integrating orthodontics into your practice is gathering the necessary data and assimilating this information in an organized fashion.

The following collection should aid all practitioners in this information gathering process. The diagnosis and treatment recording forms will serve as guidelines to streamline your overall treatment planning and treatment sequencing.

I would like to thank Dr. Joe Frank Maltsberger of Oologah, Oklahoma and Dr. David B. Uzzel of Bellbrook, Ohio for their contributions to this article. The majority of forms that are presented here are variations of our standard forms that have been edited, revised, improved and customized to better suit their individual practice needs.

These forms and letters are in part distributed to the participants of the Extended Straight Wire orthodontic program that I teach each year. These management forms can best be utilized by those people familiar with the extended course and our particular method of treatment options and sequencing. However, these same forms and letters can be customized to fit into any general or pediatric office that is currently practicing orthodontics.
